Help My Mother, Fight Very Rare, Aggressive Cancer - Bile Duct Cancer / Cholangiocarcinoma.

My name is Arvind Vats (son of Navejiven Vats), and I am reaching out to seek support for my mother, Anjana Vats, who is bravely fighting cholangiocarcinoma, a very rare and aggressive bile duct cancer. She has been undergoing intensive chemotherapy (gemcitabine 1500mg inj. and cisplatin 50mg inj.) and immunotherapy (durvalumab 1500mg inj.) for the past 3-4 months at Max Super Speciality Hospital, Vaishali. While we recently received the hopeful news that her latest PET CT scan shows a positive response, her journey is far from over. Durvalumab—the immunotherapy drug that offers the best hope for long-term survival—comes at a cost of around 6 lakh INR per session. Despite our best efforts, we’ve exhausted all our savings and resources, having already spent nearly 35 lakh INR. To continue her treatment, we need an additional 32 lakh INR, an amount we cannot manage to afford ourselves.

When my mother was first diagnosed with cholangiocarcinoma in late 2022, she underwent complete removal of the tumor. She was one of the few lucky ones to be diagnosed at a very early stage, followed by chemotherapy and radiation therapy. She remained completely cancer-free for over a year, but in July 2024, her routine PET CT scan showed a recurrence of cholangiocarcinoma. With bile duct cancer, there is roughly a 70% chance of recurrence.

She is now on this new treatment plan of - chemotherapy (Gemcitabine + Cisplatin) and immunotherapy (Durvalumab). According to the TOPAZ-1 trial, a very recent study with groundbreaking results, only 26.7% of patients respond to this treatment regimen. My mother, Anjana Vats, is one of those fortunate few who have responded to the treatment.

Unfortunately, we have exhausted all our financial resources, as this treatment has been extremely taxing and has depleted our savings. We urgently need support to continue her treatment, as immunotherapy, when effective, can provide long-term survival and potentially cancer-free outcomes.

[UPDATE]: Anjana Vats only had an insurance of 4 Lakhs INR which has been long exhausted and the treatment cost far exceeds it. Our estimation around 2 months ago was 24 lakhs INR. But the treatment cost, hospital admission, medical scans (PET/CT, Ultrasound, EUS-Guided FNAC Biopsy, USG Doppler etc.), diagnosis test, medicines, blood work and so on brings the estimation cost close to 30-32 Lakh INR

[UPDATE - 21st March, 2025]:

There have been a lot of complications since the last update. My mother was originally on chemotherapy (Gemcitabine and Cisplatin) and immunotherapy (Durvalumab). However, she was experiencing a lot of side effects from chemotherapy, so we had to discontinue it and continue only with immunotherapy (Durvalumab). She had almost completed her immunotherapy course (with just one session left) but last month (Feb 2025), she suffered a severe case of sepsis infection with septic shock, resulting in hypotension (very low blood pressure), thrombocytopenia (low platelet count), anemia (low hemoglobin), and deranged LFT and KFT.

She also experienced episodes of seizures and was in the ICU, followed by the ward, for around 16 days. We found out that this was likely due to portal vein thrombosis (a blood clot/obstruction in the portal vein that supplies blood to the liver, resulting in sepsis and pus pockets). This issue was resolved using two stent procedures, and a drainage pipe for ascitic fluid (stomach water) has been attached. This entire ordeal resulted in a hospital bill of 14.5 lakh INR.

Recently, she had another episode of sepsis infection (though likely not as severe as before) and was admitted to the ICU for 2 days and then shifted to the ward. She has now been discharged after a week, and we are still administering IV antibiotics at home with the help of a nurse. This time, the bill amounted to 3 lakh INR.

Regarding cancer, there are currently no reasons to suspect that the cancer is progressing or growing. Even in a CT scan (not a PET CT scan), there wasn't any tumor growth or indication of cancer in the report, suggesting that it is somewhat the same or hasn't significantly grown. However, the complications have been numerous, and we are trying our best to resolve them. Hopefully, things will get better
